Gum Diseases

The two major gum diseases are gingivitis and periodontitis.

Gingivitis is the inflammation confined only within the gums. The gums appear red, swollen and easily bleed. This mainly occurs due to poor oral hygiene and improper removal of plaque layer. The treatment involves regular cleaning to remove the plaque layer and instructions for proper oral hygiene measures. Gingivitis is reversible; however, if left untreated, it may progress to periodontitis.

Periodontitis is when the untreated gum inflammation progresses to include all the tooth-supporting structures, including gums, bone, and periodontal tissue. Periodontitis shows more aggressive symptoms, such as bone resorption, gum recession, and teeth loosening. Periodontitis requires a more complicated treatment course that involves non-surgical solutions through scaling and root planning and surgical solutions such as bone and gum grafts.

Periodontal Therapy Stages

Periodontal therapy is initiated with a thorough evaluation of the condition, evaluation of causes, and identifying the degree of damage that occurred.

Phase one

The first step is to stop further damage by eliminating the cause. If the reason behind the periodontal disease is poor oral hygiene, the patient is educated on how frequently to brush their teeth, the technique of brushing, and the softness of the toothbrush. Some cases may require additional hygiene measures, such as using mouthwashes. If the cause behind the periodontal disease is a medical condition, such as diabetes, it is essential to control the medical condition to prevent further damage.

Phase 2
The second step is to apply professional teeth cleaning through scaling and root planning to remove the plaque and tartar layers to decrease bacterial colonisation. Scaling is a process of removing plaque and calculus. Root planning allows for removing defective cementum from the root surface to create a smooth surface, which is favourable for tissue reattachment and healing.

Phase 3

This step involves surgical treatment solutions to reverse the damage. These solutions include pocket reduction surgery, gum graft, and bone graft. During pocket reduction surgery, the gums are retracted to properly clean the tooth surface, bacteria are removed, and finally, the gums are secured back in their place. This procedure allows for the cleaning of deep pockets. Gum graft surgery is used to restore the gum line when gum recedes due to a periodontal disease. Bone graft is a procedure aiming to restore the bone support around a tooth and prevent loosening.

Phase 4

The final phase is the maintenance phase. This involves regular check-ups to monitor periodontal tissue healing and to professionally clean your teeth to maintain favourable conditions for tissue healing.

Dr Bruno Delaunay practiced as an aesthetic restorative dentist in private practices around London, and developed his knowledge in conservative dentistry. He obtained a postgraduate certificate in Advanced Aesthetic Dentistry with Distinction from the prestigious Eastman Dental Institute

He then decided to focus on one field and completed a three-year full-time Master’s Degree in Clinical Dentistry (MCD) in Periodontology in September 2023 at the Eastman Dental Institute

During his speciality training, Bruno developed his clinical expertise in managing treatments of patients with advanced periodontal diseases using a wide range of treatments from non-surgical therapy up to complex periodontal regeneration. Bruno also acquired extensive knowledge in treating gingival recession and replacing missing teeth with dental implants

He successfully completed the examination for membership of the Royal College of Surgeons of England in 2023
