Is my treatment free?
Patients who qualify for any of the following are exempt from paying NHS charges:
• Income Support
• A valid tax credit exemption
• Job Seekers Allowance Income Based
• Pension Credit Guarantee Credit
• HC2 Certificate
• Pregnant or have had a baby in the last year
• Aged 18 and in full-time education
• Under 18
Can i get help with payments?
Help may still be available for NHS charges, particularly if you are a student, on a low income, a pensioner or receive incapacity benefit. You will need to complete an HC1 form, available from reception. Any charges you have paid may be reimbersed by DSS.
Please keep your receipts.
Alternatively, you may wish to complete the form prior to booking an appointment, in which case only the charges outlined in your DSS certificate will need to be paid.
How do i pay for my treatment?
After your consultation the receptionist will provide you with a printed treatment plan. On choosing to proceed with your care, your account becomes payable. We understand that this is not always possible. if you wish to pay as your treatment progresses you will be asked for a payment at each visit before entering the surgery to cover the cost of treatment planned for the day.
Payments can be be made by any of the following:
• Cash
• Debit/credit cards
• Cheques (cheques will only be accepted with a valid guarantee card)
Appointments
All new NHS and private patients welcome
Appointments can be made over the phone or in person.
If you prefer to be seen by a particular dentist, please notify us and we will do our best to meet your needs.
An SMS text message will be sent to your mobile phone as a reminder.
If you are late we will try and keep your appointment open but if there is not enough time left to carry out your treatment you will need to reschedule.
Cancellations
Please give 24 hrs notice if you need to cancel, so that your appointment can be given to another patient in need of urgent care.
If you fail to give reasonable notice of a cancellation you will be required to leave a refundable deposit before you are given another appointment.
All certificates regarding registration, qualifications, infection control, etc., can be viewed on request at the surgery.
